Step 3: The Form for Tracking the Flow of Attention

By clicking HERE you can download the form for tracking your experience with the exercise.  Once you’re at the form you can click the back button on your browser to return to the instructions.  If you can’t download the PDF file, contact me and I’ll send it to you as an email attachment or, if you need, in old fashioned snail mail.

The form is on 4 sides.  It may look like a lot of stuff that you’ve got to do but the “busyness” of it is simply a lot of options.  The instructions are on the first page.  On a daily basis, you only have to enter a number (the number of times you did the exercise that day) and click a bunch of boxes which describe your experience.
